When you or someone in your family has a serious medical issue, it becomes an issue for everyone in your family. That’s why continuum of care is so important. If you or your loved one live in a small community, a serious illness or injury may mean receiving treatment and hospitalization out of town. But when it’s time to go home, receive further skilled care, or undergo rehabilitation, being out of town can be traumatic for both patient and family. That’s where we come in.
